    CHA and Unite Us Expand Partnership Aimed at Improving Health Equity

    GREENWOOD VILLAGE, COLO. – Aug. 9, 2022 – Colorado Hospital Association (CHA) and Unite Us, one of the nation’s leading technology companies connecting health and social care, are expanding their partnership to advance health equity in Colorado further. Through the partnership, hospitals will have access to insight solution tools that help assess patients’ unmet social needs, empowering care providers to address their patients’ social determinants of health directly.

    The Centers for Disease Control and Prevention (CDC) describe social determinants of health as “conditions in the places where people live, learn, work, and play that affect a wide range of health and quality-of-life risks and outcomes.” Understanding social determinants of health provides greater insight into social factors that contribute to a patient’s health, and solutions from Unite Us allow hospitals to address those factors by helping connect patients with other resources in the community.

    CHA will use the Unite Us social risk score to proactively assess socially underserved populations across the state and provide data to hospitals. This will help hospitals refine quality improvement efforts to more precisely target health equity and social care gaps. With the addition of the Unite Us Platform and coordinated care networks, hospitals can further act on these data insights and securely connect patients in need with community-based resources in a streamlined, transparent manner.

    Unite Us provides a central point of contact where health care providers, social service organizations, and individuals can work across sectors to securely access and refer people to needed services while monitoring progress and measuring outcomes. CHA and Unite Us will collaborate to streamline reporting, explore electronic health records data integration, and make centralized access to Unite Us solutions available to Colorado hospitals.

    “Our collaboration with Colorado Hospital Association aligns with our goal to connect individuals and families to much-needed health and social care services,” said Kurt Waltenbaugh, Unite Us senior vice president of insights and growth. “Unite Us will provide access to predictive analytics to understand patients’ needs better and identify those with the highest social risks. Then providers can proactively connect them to resources in the network. This collaborative effort is a perfect end-to-end example of how the Unite Us Platform is leveraged to standardize whole-person care across the state of Colorado,” he added.
