Custom Data Requests

Custom data reports are available for CHA members, researchers, and other organizations. Data reports are de-identified and comply with all the requirements of the Health Insurance Portability and Accountability Act (HIPAA) for minimum necessary. Follow the steps below to request a custom data report.

1. Fill out the data request form.
2. A member of the CHA data team will be in touch to get a more comprehensive idea of your request and scope of project.
3. Your completed data request will be reviewed by CHA’s data review committee.
4. If approved, estimated cost and turnaround time will be shared with you.
