    ChartSpan and Colorado Hospital Association announce partnership to enhance preventative care for Medicare patients

    Greenville, South Carolina – May 29, 2025 – ChartSpan, a preventative care service provider, has partnered with the Colorado Hospital Association (CHA) to offer comprehensive care management programs to Medicare beneficiaries throughout Colorado. 

    CHA represents more than 100 hospitals and health systems with a shared goal of improving health and healthcare across the state. CHA Shared Services is a subsidiary of CHA that supports Colorado’s hospitals through a broad array of strategic services. By collaborating with ChartSpan, CHA Shared Services will be able to offer care management programs to hospitals and health systems who wish to expand their preventative care services. 

    ChartSpan is currently the largest full-service Chronic Care Management (CCM) organization in the U.S., serving 175+ health systems and tens of thousands of Medicare beneficiaries. Building on their experience with CCM, they recently launched a service offering for CMS’s newest care management program, Advanced Primary Care Management (APCM). 

    Chronic Care Management and Advanced Primary Care Management empower Medicare patients to manage their health through dedicated care managers, personalized health goals, and a 24-hour care line. Chronic Care Management is focused on Medicare patients with two or more chronic conditions, while Advanced Primary Care Management is available to all Medicare patients and has additional features, such as targeted support after hospital discharges.
