FCC Rural Healthcare Program – Frequently Asked Questions
Service Provider Identification Number. Vendors will need to obtain a SPIN in order to participate in this program: Obtain a SPIN Complete the required sections to participate in the Rural Healthcare Program. CHA Broadband Services works with health care providers in CO, KS, MT, WY and NM.
It does not have to be approved by the RFP deadline, but in order to participate in the program, all vendors must have a SPIN number to receive payment from USAC. http://www.usac.org/sp/default.aspx
NOV 30: All member Service Agreements and USAC Letter of Agency’s are to be finalized prior to the following funding window.
DEC 31: Member inventory lists must be submitted to Broadband Services. These lists should include all current and new services starting July 1.
JAN 2: This is the first date for USAC to review an RFP. On average, USAC will post an RFP in 10 days.
JAN 1-31: RFPs are posted by Broadband Services.
MAR 1-31: RFPs are evaluated with members and contracts are signed with service providers.
APR 1-30: Broadband Services begins filing Form 462’s (the official application request for funding).
MAY 1: Last day of the funding window.
JUL 1-JUN 30: USAC funding year.
