HCA HealthONE Presbyterian St. Luke’s – Women’s and Children’s Navigation Services
To better educate patients and families on what to expect during pregnancy and to provide support for those experiencing high-risk pregnancies, HCA HealthONE Presbyterian St. Lukes established the Women’s and Children’s Navigation Services program.
This program was developed specifically to bridge gaps in care and to support patients who were classified as high-risk. The program provides in-depth education regarding pregnancy, diagnoses, possible complications, etc. to patients and their families and connects them with appropriate specialists and follow-up care as needed.
Some successes include:
- Enhanced communication with patients and family members to provide explanations and set expectations of special care needed during high-risk deliveries
- Inceased patient return rate to the hospital for subsequent deliveries due to the support and connections offered by this program
