Advance Directives
Your Right To Make Health Care Decisions is provided by CHA as a public service to the community. The booklet informs patients of their right to make health care decisions, including the right to accept or refuse medical treatment, and provides ready-to-use forms on a variety of subjects.
These forms, and any written instructions a patient makes ahead of time about their medical treatment, are referred to as “advance directives.” The following advance directives and related subjects are covered in this booklet:
- Substitute Decision Makers: Medical Durable Power of Attorney, Proxy Decision Maker, Guardians
- Living Will
- Cardio-Pulmonary Resuscitation (CPR Directive)
- Medical Orders for Scope of Treatment
